Note: Please know, the information submitted to LCLV is confidential; however, submission of this form does not mean that LCLV represents you, as that is subject to final income verification and an in person meeting with one of our staff attorneys to review and approve your case. If approved for representation, you will then be asked to sign a retainer agreement with the agreed upon terms of representation and your initial filing fees. As a smaller nonprofit, we are limited in the number of cases we can take but do our best to accommodate all those who meet our qualification criteria.
